We are pleased you are a Delta Dental subscriber. This website will provide you with information about your dental benefits plan. If you have questions, you can contact Delta Dental of Illinois' Customer Service Department at 800-323-1743 (hours are 7 a.m. to 7 p.m. Our automated phone system, which provides network dentist information and claim status is available 24/7).

Your dental coverage includes a Dual choice monthly switch Option between Delta Dental PPO and DeltaCare dental HMO. With this plan, you are able to switch between the PPO and dental HMO on a monthly basis. To switch, you must contact Delta Dental of Illlinois' Customer Service Department by the 15th of the month (the switch is effective the first of the following month). You can contact Delta Dental of Illinois' Customer Service Department at 1-800-323-1743 (hours are 7 a.m. to 7 p.m.). You also have vision coverage through TruAssure Insurance Company, an affiliate of Delta Dental of Illinois. Please read this welcome letter for more information.

Once you enroll, Delta Dental ID cards will be mailed to your home address. You should receive your Delta Dental ID card within 10 days after enrollment. You can also print a temporary ID card through the Subscriber Connection (see below for information on how to register for the Subscriber Connection).

If you enroll in the TruAssure vision plan, your TruAssure vision ID card will be mailed to you once the open enrollment session ends. Your vision provider needs the primary enrollee's name and date of birth to confirm benefits.

If you are paying for the premiums electronically with a bank account number, please allow five business days for the enrollment to be activated.

If you are paying for the premiums with a check via US mail, please allow 10 business days for the enrollment to be activated.
